Adding Questions and Responses
Overview
Assessments and surveys are developed and maintained in the Assessment Builder.
This user guide walks through the steps of adding questions and responses to a new assessment.

When you need to create a new assessment.
* Questions must be part of a block, and one block is required to enter questions. * If your organization uses custom codes for assessments, we recommend adding a custom questionnaire code for reporting purposes. An example format is your organization's three letters appended to the assessment name. For example: **ABC-Obesity** * Custom codes for questions are also recommended. For example: **Smoking \_Cessation**
Adding a Block to an Assessment
If you have not already added blocks to the assessment, do the following:
In the Assessment Builder Dashboard, go to the assessment. You can find this quickly by using the search.
Click on the assessment and select **Add new block** to begin creating questions.
In the new block window, the Block Name is required. Reference and Custom Code are optional. Click **Create.**
_You can create multiple blocks at this step if desired, this may assist in organizing the sections and questions for larger assessments._
About Questions
There are five different formats a question can take within an assessment:
Only select one response to a question from a predetermined list of responses.
Select one or more responses to a question.
Type the response instead of selecting it. Responses to free text questions can be up to 1,000 characters, and they can contain numbers, decimals, and special characters.
Record a date in **MM/DD/YYYY** format.
Record a numeric value, such as height or weight.Responses in this field can be up to three numbers and one decimal place. For example: 175.5
Required Questions
To require a response for a question, turn on the Required toggle for that question. If a response is optional, turn off the toggle.
Entering Questions and Responses
Click **Add first question.**
Fill out the question information:
* Select the **Question type** from the drop-down menu. * _(Optional)_ Enter a **Custom Code**. * Maximum Length: 255 characters * **The Question ID** is automatically generated. * _(Optional)_ Enter a **Reference**. * A Reference is displayed on the question line in Care Management. It can include evidence-based guideline links such as [https://www.cdc.gov/diabetes/living-with/managing-sick-days.html](https://www.cdc.gov/diabetes/living-with/managing-sick-days.html). * This can be used to display instructions for the user who is entering responses. * Maximum Length: 2500 characters * Enter the **Question**. * Maximum Length: 300 characters
Add responses for the question. A question cannot be saved without at least one response added.
* Maximum Length : 200 characters * _(Optional)_ Scoring can be added to responses for use as a calculation to drive a next step such as PHQ2, or responses can be scored simply based on your unique business need. * (_Optional)_ Click  **Custom Code** to add a custom code for the response. * Click  **Delete** to remove the response.
_If you are using the Free text, date, or number question type, there will not be a responses section._
As you enter a response, the next line for the next response automatically generates. If you do not need it, continue on.
To save the question and responses, click  **Save**.
To add another question, click **Add new question.**
To discard the current edits, click .png>) **Dismiss Changes**, and then click **Dismiss** when prompted.
Saving and Publishing the Assessment
After all the questions and responses are entered, you can either save the draft for potential additional edits or publish the assessment.
Saving the Assessment as a Draft
To save the assessment as a draft, click **Close** in the lower-right corner. This returns you to the Assessment Dashboard.
On the Assessment Dashboard, search for the assessment. The status is Draft, and the assessment is not available in Care Management.
### Publishing the AssessmentTo publish the assessment and make it available in Care Management, click the  **More** button on the left panel, and then select **Publish**.
A message displays indicating the assessment has been published. Click **Close**.
In the Assessment Dashboard, the assessment is active and available in Care Management.
