Editing an Assessment
Overview
You can update questions and responses in an assessment after it has been published. This creates a new iteration of the assessment. You can view the answers saved from previous iterations in the assessment’s response history. This user guide explains how to edit questions and responses in an existing assessment.

This guide reviews how to edit questions and responses within an existing published, active assessment. Updating other functions, such as scoring or logic, are documented separately.
Deleting a question deletes child (branching) questions. To keep the branching questions, you must first move them to another parent question.To view or test edits to an assessment in the user application, the assessment must be published. It is strongly encouraged to use a non-production environment for testing changes.

Getting Started
For each of the editing activities listed above, start by doing the following:
Use the **Search** bar to find the assessment you want to edit.
After you find the assessment, you will be in the Assessment Builder.
Click **Edit** in the lower-right corner.
Editing Verbiage of Questions and Responses
Select the question, and then click the **pencil**  icon.
You can update any or all of the fields listed below:* Question Type * Custom Code (edits may impact reporting) * Reference * Question * Responses
After completing your edits, click the **check box** to save your changes. In this example, the question was modified.
Here is an example of a revised response.
_If you do not want a response, delete it and add a new response rather than changing the description of an existing response. Changing the response content to something completely different may impact the integrity of response history._
After completing your edits, you can **Close** the assessment and keep your changes unpublished.Alternatively, to make the changes available in the user interface, click **More**  and then **Publish** to publish the edits.

To discard edits, click **Cancel**  .
Adding New Questions and Responses
If you have multiple blocks, go to the block where you want to add the question. If there is only one block, scroll to the end of the assessment. Click **Add new question**.
_Click **Collapse**_  _at the end of each question to collapse the responses for a consolidated view of all the questions._
Add your question(s) and responses, and then click **Save**  .
To add responses to an existing question, select the question and click **Edit**  .
Add responses and click **Save**  .
After completing your edits, you can **Close** the assessment and keep your changes unpublished.Alternatively, to make the changes available in the user interface, click **More**  and then **Publish** to publish the edits.

To discard edits, click **Cancel**  .
Deleting Questions and Responses
If you need to delete a parent question that has child questions but want to keep the child questions, you must move the child questions to another parent question before deleting the parent question. If you do not need to keep the child questions, they will be deleted automatically when you delete the parent question.
If you have multiple blocks, go to the block where the question and/or responses will be deleted. Collapsing the questions can be helpful for a better view of all the questions.
To delete a question, click the **Delete**  **icon** on the question row.
_In this example, the child question 4.1 will be deleted as well._
You will be prompted to confirm the question deletion. Click **Delete** to continue. Click **Cancel** to keep the question.
The parent and child questions are deleted.
To delete a response, click **Edit**  to edit the question.
Click **Delete**  next to each response you want to delete.
Click **Save**  to save the changes.
When all your edits are complete, you can choose to **Close** the assessment and keep your changes unpublished.Alternatively, to make the changes available in the user interface click  **More** than **Publish** to publish the edits.

Moving Questions and Responses
To move questions and responses in Assessment Builder, drag and drop the questions into the desired order. It is recommended to collapse your questions for easier maneuvering when moving questions within a block.
_You cannot move questions from one block to another._
To move a question, hover your mouse to the left of the question number until you see the **Move**  icon.
Click and hold **Move** , and then drag the question to its new location. While moving a question down, a line displays when you are in an area where the question can be moved. (In this example, we are moving Question 1 below Question 3).
When the question is in the appropriate position, release the left button.
When moving a question up, there is no line. Release the mouse when you have the question in the position you want. (In this example, we are moving Question 2 above Question 1).
Child questions can be moved in the same manner. Collapse the questions for a better view, then use the mouse to move the questions around.
_Moving child questions to the wrong position may set the child as a parent question. The questions can be moved again to the correct position. Drag the question to the correct position._For _example, Question 3 is a child of the Yes response to Question 2. Dragging it up to the Yes will move it to the appropriate position._
To move responses within a question, click **Edit**  on the question line.
The **Move**  icon is to the left of each response. Click and hold **Move** , and then slowly drag the response to its new location.In this example, we are moving the response **“Other”** to the bottom of the list above “Unsure/unknown”
After completing your edits, click  to **save** the changes.
After all your edits are complete, you can choose to **Close** the assessment and keep your changes unpublished.Alternatively, to make the changes available in the user interface click **More**  then **Publish** to publish the edits.

Viewing the Version History
As you update assessments, a version history is maintained in Assessment Builder. The version history helps you understand what changes happen and when, and which version is currently active. When searching for an assessment in the Dashboard, there is a note next to each active assessment with unpublished changes.
Select the assessment, and then click Expand

In this example, there have been several changes to the assessment:
Not Active: Previous version of the assessment.
Active: Version of the assessment that is currently live in the user application.
Draft: Changes were made and have not been published. These changes are not visible in the application until you publish.
Click the history instance to see the assessment as it was at that point in time.
For example, the Not active version only has one question under the Oxygen/DME/Home Health Block.
The Active version displays updates that were made and published.
Additional updates are reflected in the Draft version. However, since this is not yet published, the updates are not in the application. After all the updates are complete, the draft version can be published.
Deactivating an Assessment
If there is a need to remove an assessment from the user application, you can deactivate the assessment in Assessment Builder.
Select the assessment from the dashboard.
Click **More**  **.**
Select **Deactivate**.
A prompt displays asking you to confirm you want to deactivate the assessment. Select **Deactivate**.
A success message displays.
The assessment status is changed to **Not Active.**
To locate an assessment that has been deactivated, you must check the **Show Not Active** box in the dashboard when doing a search.
To make the assessment active again, select the assessment, and then select **Edit.**
The assessment will move back to **Draft** mode for edits to questions and responses. If no updates are needed, click **More**  , and then click **Publish**.
The assessment is active.
