Branching Questions
Overview
In this user guide. you will learn how to add child questions to branch out a child question in an assessment.

When creating an assessment with branched questions based on answers.
* This document assumes you have reviewed [creating-an-assessment.md](creating-an-assessment.md "mention"). * It is recommended that you outline your questions, including branching questions, before building the assessment.
Adding Child Questions to a Question
When adding or editing a question, do the following for each child question you want to add to the question:
Click the **Add child question** branch on the right side of the page.
Fill out the question information:
* To require a response for a question, enable the **Required** toggle for that question. If a response is optional, disable the toggle. * Select at least one answer from the **Show child question if answer is any of the following** menu. * Select the **Question type** from the drop-down menu. * _(Optional)_ Enter a **Custom Code**. Maximum Length: 255 characters
While the Custom Code field is not required, it is recommended for reporting purposes. Additionally, it is recommended to use underscores (\_) instead of hyphens (-) for better readability in reports.
* The **Question ID** is automatically generated. * _(Optional)_ Enter a **Reference**. * A Reference is displayed on the child question line in Care Management. It can include evidence-based guideline links such as [https://www.cdc.gov/diabetes/living-with/managing-sick-days.html](https://www.cdc.gov/diabetes/living-with/managing-sick-days.html). * This can be used to display instructions for the user who is entering responses. * Maximum Length: 2500 characters
Enter the child **Question**. Maximum Length: 300 characters
Add **Responses** for the child question. A child question cannot be saved without at least one response added.* Maximum Length : 200 characters * _(Optional)_ Scoring can be added to responses for use as a calculation to drive a next step, or responses can be scored simply based on your unique business need. * _(Optional)_ Add logic when you want to create a plan of care problem, send a notification, create a task, or run an auto workflow rule based on the answer to the child question. * (_Optional)_ Click  **Custom Code** to add a custom code for the response. * Click  **Delete** to remove the response.
_If you are using the Free text, date, or number question type, there will not be a responses section._
* As you enter a response, the next line for the next response automatically generates. If you do not need it, continue on.
To save the child question and responses, click  **Save**.
To add another child question, hover between the child question and the next parent question (or the larger Add new question button), and then click **Add new question**.
