Adding Logic

Overview

Add logic to a question in an assessment when you want to create a plan of care problem, send a notification, create a task, or run an auto workflow rule based on the answer to the question.

When creating an assessment with branched questions based on answers.

* This document assumes you have reviewed [creating-an-assessment.md](creating-an-assessment.md "mention"). * Add the answers to the question before setting up the logic. * It is recommended to identify the questions that will tie to actions before building the questions in Assessment Builder.

Adding Logic to a Question

When adding or editing a question, click the **Add logic** cog on the right side of the page.

Use the **Setup logic** panel to add logic to the question. You can add several logic items to a question.

Select at least one answer from the **If answer is any of the following** menu.

Options in the Setup logic panel differ depending on the selected action:* **Create Plan of Care for a Problem:** Select a plan of care problem from the **Select a problem** menu. * **Send Notification:** Enter the **Notification Title** and **Notification Text**. To set the notification as urgent, select **Urgent**. * **Create Task:** Select the **Task Priority**, **Task Type**, and **Task Reason**. * **Run Auto-Workflow Rule:** Select a rule from the **Select a rule** menu.

To add more logic, click **Add more logic**, and then complete a second set of selections. To delete logic, click ![](../../../.gitbook/assets/Button_Delete_thick-gray.png) **Delete**.

When you are finished adding logic to the question, click **Save**.
