Managing Assessments
Overview
There are several classes of questionnaires in the platform: surveys and assessments. Surveys are member-facing and available through the member app. Assessments are available to users of Care Management. They are used to evaluate the member’s physical, mental, and social well-being. Medecision offers standard assessments along with the ability to configure custom content.

When you have to evaluate a member within the scope of a program using a prescriptive group of questions and need to see how responses trend over time.
* When an assessment is viewed in the context of a member’s program, it is saved within that program and is not able to be removed. * Responses in a finalized assessment cannot be changed. Recommend reviewing all responses for correctness and completeness prior to finalization.
Adding a New Assessment
Assessments are added to a member’s program. To begin, you must be on the Program Details page.
Click the **Assessments** tab in the **Program Journey** section.
Click **+ Add Assessment**.
_Alternate:_ If assessments or surveys exist for this program, the **+** (**Add)** button displays in the top-right corner of the panel.
There are three classes of questionnaires available:* **Program Assessment:** Only visible to care management users. These assessments cannot be used in a digital journey. * **Member Survey:** Made available to a member via the member portal. The Guided Health Journey is configured to automatically send after opt-in. Additional surveys may be selected and made available to the member. * **Periodic Check-in:** Made available to a member via the member portal. The Guided Health Journey is configured to automatically send on a regular period. Additional check-ins may be selected and made available to the member.
If there are existing assessments that are **not completed** on the program (status is Not Started or In Progress), the assessment will have a .png>) lock icon, and will be unavailable for selection.
Click **Create**. If you chose to create a Program Assessment, it displays.
Working an Assessment
There are multiple type of questions within an assessment:
Single-select: Only one answer can be chosen. This is indicated by radio buttons.
Multi-select: Zero to many selections can be made. This is indicated by checkboxes.
Free-text: Counter indicates the maximum number of characters that can be entered.
Date: Date can be typed in or selected from the calendar picker.
As respondents provide answers in the assessment, the completion percentage updates accordingly. There may be conditional questions within the assessment, which display additional questions based on the selected response. The completion percentage automatically adjusts to include these new branches.
Assessments are auto-saved with each answer provided. You can leave the assessment screen at any time and return without losing any previously answered responses.
Additionally, assessments may be divided into question groups. Clicking on the group header bar reveals all questions. The counter on each header indicates the number of answered questions within the group and the total number of questions. If a branch is taken, these counts automatically adjust.
If a selected response in a single-select or multi-select question is a trigger for a plan of care problem, the proposed problem description displays. The problem is generated only after the assessment is finalized.
Completing an Assessment
To complete an assessment, all required questions must be answered.
Click **Finish** on the lower-right corner of the assessment page.
In the confirmation box, click **Complete** to finalize the assessment.
_Alternate_: Click **Cancel** to return to the assessment and provide more answers or to review.
Completing a Reassessment
A reassessment can be performed if you want to re-evaluate the person after some time has passed to determine progress made while in the program. Reassessing lets you provide new answers to the same questions and compare them to the original version.
On the Assessments list in the Program journey tab, locate the _Completed_ assessment to be reassessed.
Select the arrow  icon.
Click **Reassess** on the bottom-right corner of the assessment view.
In the confirmation box, click **Confirm**.
**Alternate:** Click **Cancel** if you would like to view the completed assessment instead.
Viewing the Assessment History
History Mode allows you the ability to track and analyze the historical changes of the assessment answers over time and view them in a chronological format. If this assessment was performed on a different program, those responses will display here. The default upon beginning will be “Current” plus the latest 3 assessments (if applicable).
You can revert back to the traditional method for adding responses by selecting the “Questionnaire Mode” button at any time, and all responses from history mode will remain. This allows you to view any proposed problems or tasks that might be triggered from a response.
From the Program Journey section, click the **Assessments** tab.
Either add a new assessment or select an existing assessment that has more then one entry.
Click **History Mode**.
History mode will display the current responses (if the assessment is **In Progress**) and the three most recent assessments. In this view, you can add or update new responses while viewing the previous.
You can view **completed** assessments in history mode if you are not doing an update. Select the completed assessment, and then select **History mode**.
Click **Chronology Setting** to change the view. Review the date/time stamps and add or remove the records you would like to review by checking/unchecking the boxes.
If more then current + three are selected, you can use the scroll icon to move the responses to the right and left.
Add new responses under the **Current** column.
Assessments are auto-saved as they are entered, so if you are unable to complete the assessment, you can go back to where you left off.
To finalize the assessment, click **Finish.**
Click **Complete** to confirm you want to finalize the assessment._(Alternate)_ Click Cancel.
While in history mode, you can return to the traditional Questionnaire Mode at any time by clicking the button and continuing your workflow.
Viewing the Response History
Responses to completed assessments can be reviewed at any time. If there are multiple instances of the same assessment, as seen with reassessment, all answers can be reviewed for one question.
On the Assessment list in the Program journey tab, locate the most recent _Completed_ assessment for which you want to review the responses, and then click on the arrow .png>) arrow.
Click **Response History** .png>) for the question you want to view.
Click **Close** to return to the assessment.
Viewing the Assessment Scoring
Assessments may have been configured with scores. The value is calculated based on responses as provided. The score is calculated as responses are provided and saved after the assessment is finalized.
An assessment with only one score displays the running value on the bottom of the page during assessment entry.
Hover over the info icon to display the meaning of the current score.
For an assessment with multiple scores, click **Scores**.
A box displays the **Index Name** and **Score**.
Hover over the .png>) information icon to view the definition of the score.
Click **Close** to return to the assessment.
All questions that contribute to a score are required fields, however, they may not be marked as such. This rule ensures that the entire calculation can be completed.
Viewing an Assessment Score from the Program Journey
If your assessment is configured to calculate scores, you can view the score of a completed assessment from the Program Journey.
Go to a Program, and then go to the **Assessments** tab under the Program Journey.
Click the **Score Calculator** .png>) icon for the assessment.
Click the **Information** .png>) icon to view the score descriptions.
If there are multiple scores from one assessment, it will display in a similar manner.
Click **Close** to return the the assessment list.
