Completing Surveys
Overview
Surveys collect information about a person. Surveys provide valuable data that can influence decision-making and help customize the care journey experience to a person's needs.
This guide explains how to access and complete a survey from the Member Application.

When you would like to complete a survey that has been assigned to you within the member app.

Accessing a Member Survey
Surveys are configured by your organization and related to the program in which you are enrolled. When you log into your Member App Dashboard, a survey may be available.
Log into the **Member Application**.
The Landing Page of the Dashboard includes Surveys.* The **Surveys box** indicates that there is a **New** survey. * The **New Survey** section of the page indicates there is a new survey available.
Click on the survey to begin.
If you haven't completed your survey in the time configured by your organization, you will receive a text or email reminder letting you know that you have a new survey to complete.
Completing the Survey
Click **Next**.
Select your answer, and then click **Next**.
The **Back** button allows you to navigate to the previous page. The **Step** shows you where you are in the survey.
After you finish answering the questions, the Survey Accomplished page displays.
To return to the dashboard, click **Return to the Dashboard**. Your completed survey will be marked as **Done**.
To view your completed survey, click on it.
