Converting a Program
Overview
Converting a program may be necessary under the following circumstances:
Changing Needs: When a member's healthcare needs evolve but the current program no longer adequately addresses them.
Program Availability: If a new program becomes available that better suits the member's goals and preferences.
Better Fit: When the Guided Health Journey offers a more comprehensive digital engagement than the Classic program.
Technology Upgrade: If technology tools like apps and chat features introduced in a Guided Health Journey are required for enhanced member experience.
There are two major classes of programs in the platform: Classic (Telephonic Only) and Guided Health Journey (sometimes called a Digital Journey). A Guided Health Journey is a program that can be conducted telephonically and includes digital capabilities, such as a member app and chat.
Both classes of programs allow you to covert a program. Converting a program changes the current program to the new one selected. The current program is closed, and a new program is opened. The old and new programs are linked to each other.
* Access to Care Management * Permission to create a program
When you determine that a member's current program should be changed to better suit their healthcare needs.
Follow your organization’s policies and procedures when converting a program for a person. This includes such considerations as the person’s coverage options, who can open a new program, and how to assign program ownership.
