Educational Materials - EMMI
Overview
This document guides you through adding an EMMI engagement within a plan of care. EMMI content contains different types of educational resources for engaging a person, providing multiple sources for learning, and empowering the person to manage their condition effectively and improve their overall health outcomes.
* Access to Care Management * Permission to add/edit Plans of Care * Organization has access to the EMMI Engagement Library
When you identify a need for additional education for a person, you can create a plan of care problem to include EMMI engagements.
* When selecting a new problem or intervention, ensure the word **EMMI** is included in the selection. * When selecting a goal, include an action the person should take, such as viewing a problem.
All plans of care are tied to programs. You can add, edit, and view plans of care only from within a program.
Opening a New Plan of Care Using EMMI
From the Program Details page, select the **Plan of Care** tab in the Program Journey section.
Click the **Add Problem** button.
_(Alternate)_ Click the plus
icon. This button displays when there are already one or more problems listed in the Plan of Care section.The New Problem page displays. From here, selections can be made from drop down menu’s as well as selecting a Target Completion Date for the Problem
The status field is greyed out and cannot be changed.
Select a **Problem Description** from the menu. Emmi materials have (emmi) in the description.
It is crucial that your selected problem have (emmi) in the description. Otherwise no associated emmi goals and interventions will be available.
_(Optional)_ Select a **Priority** for the problem.
Review the **Start Date** and update if desired. The Start Date defaults to the current date.
_(Optional)_ Select a **Target Completion Date**.
Click **Create**.
The Plan of Care page displays the new problem with the details you selected.
Opening a New Goal Associated with EMMI
From the Plan of Care page, click **Add Goal**.
Select the **Goal Description** from the menu. Ensure the goal includes Emmi in the description.
The selected goal must contain Emmi in the description in order for the Emmi intervention to be available.
Choose the **Type** of goal: Long Term or Short Term
This choice automatically sets the value of **Target Completion Date** per your organization’s configuration.
Review the **Start Date** and update if desired. The Start Date defaults to the current date.
_(Optional)_ Select a **Target Completion Date**.
_(Optional)_ Select a Focus for the goal: **Caregiver**, **Member**, or **Provider**.

Click **Create**.
Adding an Intervention
Click the **Maximize**
icon to expand the goal section.
Click the **Add Interventions** button on the Interventions tab.
Select the **Intervention Description** from the menu. Ensure the intervention includes the **EMMI** tag to the right of the description.
Review the **Start Date** and update if desired. The Start Date defaults to the current date.
_(Optional)_ Update the **Target Completion Date**. The default value is two months from the current date.
Choose the **Member Email**. When you select a Label, the corresponding email address displays.
Click **Create**.
You can filter for interventions that are in progress or completed under the Interventions tab.
Viewing EMMI Engagement Activity
Click the **Educational Activity** tab.
Click the filter buttons to sort Emmi engagements by status: All, Initiated, Completed, or Expired.
The number next to the status reflects the total number of Emmis with that status. The absence of a number indicates there are zero Emmi interventions with that status tied to this particular program.
Click the arrow
icon to return to the Plan of Care page.
The new Emmi intervention displays on the Plan of Care page .
