Plan of Care Goals, Interventions and Barriers

A Plan of Care provides a framework for reaching better health outcomes while managing the person. After identifying problems, it's essential to establish goals, outline interventions to meet those goals, and recognize any obstacles that may impede progress.

When you are ready to document the additional components in a plan of care and after problems have been identified.

* Custom descriptions can be used for all component of a plan of care. * Custom descriptions are not able to be edited. * Interventions and barriers can be deleted. * Goals are not able to be deleted. * A goal cannot be added to a problem if it is not in Open status. If the problem is Proposed or Deferred, the plan cannot be built further.

Adding a Goal

From the Problem Details page, click **+Add Goal**.

_Alternate:_ If a goal exists in the problem already, click the **+** button in the upper right-hand corner to add another goal.

Click on the **Search Goal Description** field.

Select the appropriate goal from the list of returned values. This list is constrained to the goals mapped to the selected problem.

**Tips for Success*** There might be a slight pause as you scroll through the list as the next batch of matches are retrieved. * Goals that are added to the problem are disabled for selection and have a lock icon.

_Alternate:_ Enter a custom description by typing it into the **Search Goal Description** field.

Select a goal type. This choice defaults the value of **Target Completion Date** per your organization’s configuration.

_Optional:_ Change the **Start Date**. It cannot be prior to the problem start date.

_Optional:_ Change the **Target Completion Date**. It cannot be prior to the goal start date. Use the calendar to select a new date or use the Quick date input.

_Optional:_ Select a focus for the goal. Multiple choices can be made.

_Optional:_ Select a **Priority**. See [_Setting Priority_](plan-of-care-goals-interventions-and-barriers.md#setting-priority) for more details on the usage of priorities.

Click **Create.**

Using the Quick Date Input

The Quick Date Input feature allows you to specify a date based on today's date. For example, if you want to set a target completion date to two weeks from today, type T+2w.

When adding a new Goal or Intervention, the Quick Date Input feature is in the **Target Completion Date**.

Click into the **Quick date input** box. If you want to set a target completion date to two weeks from today, type T+2w or hover over the information

icon for additional examples.

In **Barriers**, the Quick Date input is available on the **Identification Date**.

Adding an Intervention

To begin, you must maximize an open goal using the caret on the right-hand side of the panel.

Click the **Interventions** tab.

Click **+Add Intervention**.

_Alternate:_ If interventions exist for this goal, the add button appears in the upper right-hand corner of the panel.

Click on the **Search Intervention Description** field.

Select the appropriate intervention from the list of returned values. This list is constrained to the interventions mapped to the selected goal.

_Optional:_ Change the **Start Date**. It cannot be prior to the goal start date.

_Optional:_ Type in a **Target Completion Date** or select one from the calendar picker or use the Quick date input feature. It cannot be prior to the intervention start date.

Click **Create.**

Adding a Barrier

To begin, you must maximize an open goal using the caret on the right-hand side of the panel.

Click the **Barriers** tab.

Click **+Add Barrier.**

_Alternate:_ If barriers exist for this goal, the add button appears in the upper right-hand corner of the panel.

Click on the **Search Barrier Description** field.

Select the appropriate barrier from the list of returned values.

**Tips for Success*** There might be a slight pause as you scroll through the list as the next batch of matches are retrieved. * Barriers that are added to the goal are disabled for selection and have a lock icon.

_Alternate:_ Enter a custom description by typing it into the **Search Barrier Description** field.

_Optional:_ Change the _**Identification Date**_. It cannot be prior to the goal start date. Manually enter the date, select one from the calendar picker or use the Quick date input feature.

Click **Create.**

Editing Goals, Interventions, or Barriers

Click the pencil icon associated with the item to edit

Make the necessary changes. Disabled fields are not able to be changed.

Click **Save.**_Alternate:_ Click **Close** to discard the changes and close the dialog box.

Deleting Interventions or Barriers

Click the delete icon associated with the item to remove.

Click **Delete.**_Alternate:_ Click **Cancel** to close the dialog box and not remove the item from the plan of care.

Setting Priority

Priority on a goal is a field where you can choose to assign a ranking to the goals within the problem. The ranking ranges from 1 (highest priority) to 5 (lowest priority). The following points detail the actions you can perform:

  • Each ranking can only be given to one goal within the same problem at a time.

  • Closing a goal with a ranking keeps the ranking intact.

  • The ranking from a closed goal can be applied to another goal.

If there are more than five goals in the plan, only five can have a ranking.
