Closing a Plan of Care

Overview

To close a plan of care problem efficiently, follow a bottom-up approach. For each goal, close its barriers and interventions first, and then close the goal. After closing all goals within a plan of care, you can close the plan of care.

This user guide walks you through the steps to close a plan of care problem, following the order described above.

When you have an open plan of care that needs to be closed due to member termination or member graduation from the program.

* You are unable to close an open problem until all goals associated with that problem are closed. * Complete all interventions and resolve all barriers tied to each goal before closing the goal. * After closing all goals, you can close the plan of care problem.

Accessing a Plan of Care

Go to a program, and then click the **Plan of Care** tab in the Program Journey section.

Click the **arrow** to view the open problem.

Go to the goal and click **Expand** to display the interventions and barriers.

Resolving a Barrier

Click the **Barriers** tab.

Click the **pencil** on an open barrier._Alternate:_ Click **In Progress** to display only unresolved barriers. This is helpful when there is a large list of barriers included.

Enter a **Resolution Date** or click a date from the calendar picker. The Resolution Date must be on or after the Identification Date, and it cannot be a future date.

Click **Save**.

Closing an Intervention

Click the **Interventions** tab.

Click the **pencil** icon on an open intervention._Alternate:_ Click In Progress to display only open interventions. This is helpful when there is a large list of interventions included.

Enter an **Actual Completion Date** or click a date from the calendar picker. The Actual Completion Date must be on or after the Start Date and it cannot be a future date.

Click **Save**.

Closing a Goal

A goal can be closed after all dependent interventions and barriers are completed or resolved.

Click the **Close** icon for the goal.

If you overlooked resolving a barrier or closing an intervention, an error message displays and you are not able to continue with closing the goal until those actions are completed.

Select the appropriate **Outcome**.

_Optional:_ Change the **Actual Completion Date**. This date must be on or after the goal Start Date and cannot be a future date.

Click **Save**.

Closing a Problem

A problem can be closed after all dependent goals have been completed.

Click the **Close** icon for the problem.

If you overlooked closing a goal, an error message displays and you are not able to continue with closing the problem until all goals are closed.

Select the appropriate **Outcome**.

_Optional:_ Change the **Actual Completion Date**. It must be on or after the problem Start Date and cannot be a future date.

Click **Save**.
