Plan of Care Problems
Overview
A Plan of Care provides a framework for reaching better health outcomes while managing the member. The first component is the problems the member is experiencing.
Problems are added to a plan of care in two ways:
Manually selected by the user
Automatically proposed by an assessment response
This user guide focuses on plan of care problems. Additional user guides provide more details on the additional plan of care components.

Setting Priority
Priority on a problem is a field where you can choose to assign a ranking to the problems within the plan. The ranking ranges from 1 (highest priority) to 5 (lowest priority). The following points detail the actions you can perform:
Each ranking can only be given to one problem at a time.
Proposed or deferred problems cannot be ranked.
If an open problem is deferred, the ranking is removed.
Closing a problem with a ranking keeps the ranking intact.
The ranking from a closed problem can be applied to another problem.
If there are more than five problems in the plan, only five can have a ranking.
