Plan of Care Problems

Overview

A Plan of Care provides a framework for reaching better health outcomes while managing the member. The first component is the problems the member is experiencing.

Problems are added to a plan of care in two ways:

  • Manually selected by the user

  • Automatically proposed by an assessment response

This user guide focuses on plan of care problems. Additional user guides provide more details on the additional plan of care components.

When you have a need to build a plan of care for a member.

* Once a proposed problem is opened, it cannot be reset to proposed. You will need to either Defer or Close the problem. * The same pre-defined Problem can be open in different programs for the same member. * Different assessment, same problem open, does not create a new problem, but there is no connection to the secondary assessment.

Manually Adding a Problem

Problems are added to a person's program. To begin, you must be on the Program Details page.

Go to a program, and then click the **Plan of Care** tab in the Program Journey section.

Click **+ Add Problem**.

_Alternate_: If problems exist or are proposed for this program, the **Add** button displays in the upper right-hand corner of the panel.

Enter one to several keywords in the Search Problem Description field.

Select the appropriate problem from the list of returned values.

Tips for Success:* The more you type, the more the list will narrow down to make it easier to locate the problem you want. * There might be a slight pause as you scroll through the list as the next batch of matches are retrieved.

_Alternate:_ Enter a custom description by typing it into the **Search Problem Description** field.

_Optional:_ Change the **Start Date**. It cannot be prior to the program start date or a future date.

_Optional:_ Select a **Priority**. See the _Setting Priority_ topic in this Job Aid for more details on the usage of priorities.

_Optional:_ Enter a **Target Completion Date** that is after the selected Start Date.

Click **Create**.

Quick Dates

Quick Date Input feature allows you to specify a date based on today's date. For example, if you want to set a target completion date to two weeks from today, type T+2w.

Click to add a new problem. The Quick Date Input feature is in the Target Completion Date.

Click into the Target Completion Date box. If you want to set a target completion date to two weeks from today, type T+2w or hover over the information

Opening a Proposed Problem

Proposed problems from assessment responses are triggered only after the assessment is finalized.

Click the **arrow** ![](<../../../../.gitbook/assets/image (646).png>) icon for a Proposed problem.

Click **Open**.

_Alternate_: Click the **pencil** icon to edit the problem and change the status from Proposed to Open.

Click **Save**.

Deferring a Proposed Problem

Proposed problems from assessment responses are triggered only after the assessment is finalized.

Click **Defer**.

_Alternate_: Click the pencil ![](<../../../../.gitbook/assets/image (649).png>) icon to edit the problem and change the status from Proposed to Deferred.

Editing a Problem

You are able to edit a problem to change the status, priority, or targeted completion date.

Click the **pencil** ![](<../../../../.gitbook/assets/image (648).png>) icon on the problem to edit.

Make the changes needed, and then click **Save**.

The options for Status are dependent on the problem’s current status:* Proposed: Open or Deferred * Open: Deferred * Deferred: OpenThe Problem Description, including custom problems, and the Start Date are not able to be changed.

_Alternate:_ Click **Close** to close the edit dialog box without saving any changes.

Setting Priority

Priority on a problem is a field where you can choose to assign a ranking to the problems within the plan. The ranking ranges from 1 (highest priority) to 5 (lowest priority). The following points detail the actions you can perform:

  • Each ranking can only be given to one problem at a time.

  • Proposed or deferred problems cannot be ranked.

  • If an open problem is deferred, the ranking is removed.

  • Closing a problem with a ranking keeps the ranking intact.

  • The ranking from a closed problem can be applied to another problem.

  • If there are more than five problems in the plan, only five can have a ranking.
