Surveys in Guided Health Journeys
Overview
Guided Health Journeys are configured to send a survey to the person after they log into the member app for the first time. Additional periodic check in surveys can be sent on a schedule.
Additional surveys can be manually sent to the person. The person is notified upon next member app log-in that they have a new survey available. Scheduled reminder texts or emails are automatically sent to prompt the person to answer the survey.
This user guide focuses on sending additional surveys to a person enrolled in a Guided Health Journey and how to review responses provided by the person. To perform a program assessment on the person, refer to Managing Assessments.

When a person is actively engaging in a Guided Health Journey and surveys are used to decide on next step of the care management program.
* To check on the person’s progress on the survey, you can monitor the surveys tab in the program. * After completion of the survey, notifications or tasks may be created based on the responses for you to act on. * Ensure the journey is not paused when adding a new survey
Manually Sending a Survey
The starting point is the details’ view of a Guided Health Journey that a person has opted in.
Click the Assessments tab.
Click the + icon.
Select the Member survey radio button.
Select the survey from the list.
Tips for Success* To choose a different survey after selecting one, select the X to remove the selected value. * The more you type, the more the list will narrow down to make it easier to locate the survey you want. * There might be a slight pause as you scroll through the list as the next batch of matches are retrieved. * Program assessments are not made available to the person via the member app. * The name displayed in the list is not always the name of the survey the person sees in the member app. This is configured by your organization.
Click Create
_Alternate:_ Click Close to return to the program details and abort sending a new survey to the person.
Reviewing a Person's Responses to a Survey
The starting point is the details’ view of a Guided Health Journey that a person has opted in.
Click the Assessments tab.
Click the Member Survey tab.
The list of surveys sent to the person displays. Surveys created by _Digital Journey_ are those sent automatically by the Journey Executor. The status indicates whether the person has started the survey or completed it..png>)
Click the arrow icon on a survey with a _Completed_ status
The survey displays in read-only. You can review to see which responses were provided by the person. Surveys do not contain any branching, and all questions are required to be answered by the person to complete the survey. Any tasks or notifications generated by the responses are displayed with the response.
Click the Hide Assessments icon in the upper right corner to return to the program view
_Optional:_ Click Reassess to manually send this survey to the person in their member app.
