Guided Health Journey Reference Guide
Guided Health Journeys are managed automatically as configured by your organization. There are four main sections in the Digital Journey: Opt In, Assessment, Active Journey, and Exit Journey. \ This reference guide provides you with an understanding of each state and its related actions in the journey.

Stages in the Guided Health Journey Bar
Opt In: Agreed
The person responded Yes to the SMS or email invite to enroll in a Guided Health Journey. Date and time stamp of the person’s action is displayed.
Opt In: Disagreed
The person responded No to the SMS or email invite to enroll in a Guided Health Journey. Date and time stamp of the person’s action is displayed. The Digital program is automatically closed.
Opt In: Requests More Info
The person responded Help to the SMS or selected More Info in the email invite to enroll in a Guided Health Journey. Date and time stamp of the person’s action is displayed.
If the person takes no action from the SMS or email, the program will remain in its initial state until it is automatically closed on the configured date.
Your organization can configure a task to be generated if the person requests more information. If an individual user owns the program, the task is assigned to them. Otherwise, the task is assigned to the program’s default department.\ It is possible for a person to respond Yes or No after requesting more information. If this occurs, you can view the complete history of person interactions in the Audit History.
Assessment
After the person logs into the member app, the assessment state changes to In Progress.
If the person never logs into the member app, the program will remain in the Opt In state until it is automatically closed on the configured date.
Active Journey
The Active Journey state transitions to In Progress after the person has submitted the survey configured in the journey. This occurs after enrollment at a preconfigured number of days set by your organization.
Additional surveys or education materials are automatically sent to the person as periodic check-ins, and this is also configured by your organization. If the person never completes the initial survey, the program remains in the Assessment state until it is automatically closed on the configured date.
Exit Journey
A Guided Health Journey is normally closed automatically when the configured run time expires. A Journey can also be closed by a user, if deemed necessary.
Digital Journey Tab
The guided health journey is managed automatically for the member, and you can view the message templates and settings in the Digital Journey tab in the Program Details. All stages of the digital journey are pre-configured by your organization.
There are four expandable sections in the Digital Journey: Opt In, Assessment, Active Journey, and Exit Journey.
Each section contains multiple sub-sections that display the presets configured for the program:
### Opt In: Initial outreach to the member
* **Opt In Outreach:** Displays the member outreach messages and reminders, including what happens if the member does not respond. * **Member Responds Yes:** Displays the member onboarding messages and reminders if the member responds Yes, including what happens if the member does not log in. * **Member Responds No:** Displays the member message for if the member responds No. * **Member Requests More Info:** Displays what happens if the member requests more information. For example, the care manager can receive a task to reach out to the member.
### Assessment: Initial surveys sent to the member
* **Template Delivery:** Displays the assessments the program delivers to the member by the program and when to deliver the assessments. * **Assessment Reminders:** Displays the assessment reminder messages the program delivers to the member. * **Completion Message:** Displays the assessment completion message the program delivers to the member and what to do if the member does not do the assessment after a set number of follow up messages.
### Active Journey: The core of the digital journey with periodic check-ins and educational content
* **Periodic Check Ins:** Displays the periodic check-in survey that is delivered to the member, along with the set frequency, reminders, and what to do if the member does not complete the periodic check-in. * **Additional Educational Content:** Displays the educational content (e.g. videos) the program sends to the member, along with the frequency and reminder settings. * **Pause Journey:** Displays the message the program sends to the member if the member pauses the digital journey.
### Exit Journey: The end of the digital journey
* **Inactive Member:** Displays the exit message sent to an inactive member and when the program sends the message. For example, the program can be configured to send an exit message to the member after 30 days of inactivity. * **Program End:** Displays the program completion message sent to the member.
