Managing Notifications from a Guided Health Journey

Overview

Notifications are informational messages for the Care Manager for Guided Health Journeys (Digital Programs). The platform automatically generates a notification if a member/person responds to a survey with a response configured with this action within a Guided Health Journey.

The system assigns the notifications to the department that owns the member’s program. If after reviewing the notification, the Care Manager decides further action needs to be taken, a task can be created from the notification.

When Notifications appear on a department’s worklist or an individual’s worklist.

* Notifications have a limited lifespan. The platform automatically removes the notifications that are older than 30 days from the department worklist or the individual worklist. * Notifications are not generated until the member has completed the survey via the member app.

Accessing and Viewing Notifications

You can access notifications from any of the following areas.

  1. Your individual Home page

  2. Your department’s Home page

  3. Comprehensive Person View (CPV)

  4. Program

Your individual Home page

To view notifications, go to the Notifications tab on the Home page._Note:_ The most urgent notifications are displayed first, followed by the most recent.

Department Homepage

Select the department from the drop-down menu on the Department tab listed on the Homepage

Click on the Notifications tab on the panel.

Comprehensive Person View (CPV)

To view all notifications associated with a person, go to the person’s CPV

Click on the Notifications tab

Programs

To view all notifications associated with a single program, go to the program view.

Click on the Notifications tab on the lower panel._Note:_ Notifications are not applicable to _Classic_ programs.

Filtering Notifications by Timeframe

You can filter what Notifications you wish to view by any of these timeframes:

  • Last 24 Hours

  • Last Week

  • Last Month

The timeline filter is available on all notification panels.

To filter notifications, click on the menu in the Notifications tab.

Select the timeframe for when you want view notifications.

Filtering Notifications by Program

You can filter Notifications by Program. This program filter option is available from the CPV and the individual / department home pages

Select the second drop-down menu located on the header of a Notifications

Select the program from which you want to view notifications.

Creating a Task from a Notification

If after viewing a notification, you decide that further action should be taken, you can create a task that links the notification to the task.

Click the Create Task button in the bottom-right corner of the notification tile.

On the task details screen, fill in all required fields. These are marked with a magenta square.

In the Task Description field, add a note explaining the need for the task.

_Optional: C_hange or remove the individual task owner._Note:_ The task is automatically assigned to the program’s configured default department

Click the Save & Exit button

Viewing a Task Associated with a Notification

It is only possible to create one task association for each notification. If a task has already been created, a View Task action is available, instead of the Create Task action.

Click the View Task button in the bottom-right corner of the notification tile.

Reviewing Expired Notifications

The system removes notifications that are older than 30 days. This information may still be relevant to the person’s care, so you can view expired notifications while reviewing the person’s survey history.

Click the arrow icon on a completed member survey from the Assessments tab on a program.

Responses display what notification was triggered, if any.
