Creating a Guided Health Journey

Overview

There are two major classes of programs in the platform: Classic (Telephonic Only) and Guided Health Journey (sometimes called a Digital Journey). A Guided Health Journey is a program that can be conducted telephonically and includes digital capabilities, such as a member app and chat.

There are three sub-types of programs: Medical, Pharmacy, and Behavioral Health. Each sub-type is fundamentally the same. They are distinguished in list views by the icons.

This user guide focuses on creating a Guided Health Journey. Classic programs are addressed in additional guides.

When you identify a person who should be enrolled in a Guided Health Journey due to their willingness to participate in the digital aspects of this type of program.

* Follow your organization’s policies and procedures when adding a program for a person. This includes such considerations as the person’s coverage options, who can open a new program, and how to assign program ownership. * Upon saving the program, the Journey kicks off. Dependent on the configuration, an Invitation is set to the person via SMS or email. * Guided Health Journeys close automatically after the configured time has passed.

Creating a Guided Health Journey Program

The starting point is an active person’s CPV.

Click the **Programs** tab.

Click **Create new Program**.

Select the program type._Note:_ The available type options may be limited by your functional permissions or your organization’s configuration. Follow your policies and procedures in selecting the type.

Select the Program.

Select Digital Journey._Note:_ Programs that are configured as Guided Health Journeys can be opened as a Classic program. This is equal to creating a telephonic-only program.

Tips for Success* Guided Health Journey programs are badged as Digital. * A person cannot be active in the same program at the same time. Programs that are already open for the person are not able to be selected and have a lock icon. * To choose a different program after selecting one, select the X to remove the selected value. * The more you type, the more the list will narrow down to make it easier to locate the program you want. * There might be a slight pause as you scroll through the list as the next batch of matches are retrieved.

Select the Source.

_Optional:_ Select the Severity.

_Optional:_ Change the Screening date. If cannot be in the future.

_Optional:_ Change the Department Owner. The department configured by your organization is auto selected.

_Optional:_ Change the User Owner. Your name is auto selected._Note:_ You can remove the User Owner. The program created is owned by the selected department.

Click Create.

_Alternate:_ Click Cancel to return to the CPV without creating a program.

Tips for Success: If the Create new Program button does not display on the Programs tab in the CPV, check to make sure the person is active. A quick way to see this is by looking at the dot on their initial icon. If it is green, they are active
