Creating an Assessment
Overview
In the Assessment Builder application, all assessments and surveys are developed and maintained. The process for building an assessment involves several steps: initially creating the assessment, followed by adding blocks, incorporating questions, and finally enhancing these questions with logic or branching.
This user guide illustrates the initial phase of creating an assessment or survey.

When you need to start the process of building a new assessment or person-facing survey.
* The Assessment Builder application is a stand-alone module. * It is not necessary to add any items to an assessment immediately after creation. You can save it and return at a later point to build it. * Assessments begin in a _Draft_ state. Refer to [managing-assessment-versions-draft.md](managing-assessment-versions-draft.md "mention") for more information on other assessment states.

Enter the **Assessment Name**.* Maximum Length: 100 characters
_(Optional)_ Enter the **Assessment Alias.*** Title of the assessment displayed in the member app. * This field is only applicable if you are using _Guided Health Journeys._ * Maximum Length: 100 characters
_(Optional)_ Enter a **Reference.*** Description of the assessment that displays below the assessment name in Care Management. This can be used to display instructions to the user entering responses or it can be evidence-based guideline links such as https://www.cdc.gov/diabetes/living-with/managing-sick-days.html. * Maximum Length: 250 characters
Select **Assessment Type.*** Person Level: Questions specific to the person, independent of the person being enrolled in a program. Examples are Health Risk Assessment or PHQ-9. * Program: Questions related to the program the person is enrolled. Usually involves direct contact with person via phone. Examples are Anxiety Assessment or Oncology Assessment. * Request: Questions used to capture details related to Appeal and Grievance Requests.
### Assessment Subtype
Select one to many **Subtype(s).*** Member survey * Initial assessment sent to the member during a _Guided Health Journey._ * Available in the member app and Care Management. * Periodic Check In * Assessment sent to the member during the active phase of the _Guided Health Journey._ * Available in the member app and Care Management. * Program Assessment: Assessment responded to by a Care Management user.
Select **Subtype.*** Appeal: Assessment presented during the Appeal workflow. * Grievance Intake: Assessment presented during the Grievance workflow. * Grievance Outcome: Assessment presented during Grievance decisioning. * Guided Intake: Assessment presented during the intake process to gather additional information about the services being requested.
### Configurations
* Select the programs for which this assessment is available. * Click a parent checkbox to select all children. * Use the caret to expand the parent node.
Set the profile of requests for which this assessment is available.* Click **All** or specific **Request Category** options. * Click **All** or a specific **Health Care Type** * Click **All** or specific **Request Type** options.Appeals and Grievance assessments can optionally be mapped to specific business categories, diagnoses, or services.* Select an **LOB** to limit this assessment's availability to persons with that LOB in their active enrollment. * Select a **State** to limit the assessment's availability to persons with a mailing address in that state. * Search to add diagnoses codes. More than one can be added. * Search to add procedure codes. More than one can be added. * There cannot be more than one Appeal assessment with the same configuration.
Set the profile of requests for which this assessment is available.* Click **All** or specific **Request Category** options. * Click **All** or a specific **Health Care Type** * Click **All** or specific **Request Type** options.
Enter a **custom code.** This must be unique and cannot be the same custom code ID for other assessments.
The **Assessment ID** is an automatically generated unique ID (UUID). This cannot be changed.
Slide the **Scores** toggle to include scoring logic in the assessment.
Click **Create.**
