Creating a Request from a Fax

Overview

Faxes provide supporting documentation for a request, ensuring all parties have access to key information.

A fax can have the following uses:

  • Acts as a verification tool, confirming details or authorizations required for processing.

  • Streamlines communication by consolidating all related documents in one place.

  • Helps with maintaining a comprehensive record of communications and related documents for future reference.

This user guide details the steps to create a new request from an existing fax. The process is similar to creating a new request, but with the fax attached.

When you pull a fax to process that does not yet have a corresponding request. When this occurs, you can attach the fax to a new request.

When creating a new request from a fax, the request notification date/time defaults to current date/time, this is not the date/time of the incoming fax. Refer to your organization’s workflows to determine how the notification date should be entered.

Creating a New Request From a Fax

To access the fax queue, select the **Faxes** tab on the Worklist. The default shows Unprocessed faxes assigned to the user or department.

From the Unprocessed tab, click the **Create New Request** button on the fax you want to create the request for, and then select the type of request from the drop-down menu.

_The selections in the drop-down menu may differ based on your permissions._

Enter the information for the request.* Requestor Information is optional. * Request details are required.

Click **Create** to generate the new request.

Clicking **Cancel** will cancel the new request and the fax will remain in your Unprocessed fax worklist.

The request source defaults to Fax, and it cannot be edited.The Notification Date/Time is the time the new request is initiated, and it can be edited.

After the request is created, you will be guided through the steps to enter the supporting information for the request. This includes the following:* **Basic Information:** Person and Provider(s) * **Medical Information:** Diagnosis and Services * **Additional Information:** Attachments and Review

Refer to the documents in [requests-and-referrals](../requests-and-referrals/ "mention") for details on adding the Basic Information and Medical information to the request.

In each section, you will be prompted to either Continue or Delete Draft. Click **Continue** to save and move on to the next section.

If you choose to **Delete Draft**, you will be prompted to confirm.

If you select **Delete**, the new request will be cancelled, and the fax will move to the **Processed** tab of your fax worklist.

In the Attachments section, the fax you are attaching to the new request is automatically added. To add additional attachments to the request, click **+Add Attachment.**

_(Optional)_ Edit the existing fax information:1. Click the ![](<../../../../.gitbook/assets/image (500).png>) pencil icon. Edits for the fax attachment are not required.

The date/time of the fax in Attachments is the time the fax is being attached to the new request. It is not the date/time of the incoming fax.

2. Edit the fax information as needed: * File Title and Category can be edited. * Association defaults to Initial Request, and this is the only option at this step. 3. Click **Save** to save changes, or **Cancel** to return to the new request creation.

Click **Continue**.

Use the **Review and Submit** page to review the new request before submitting it. You can edit the information in any of the sections by clicking the ![](<../../../../.gitbook/assets/image (499).png>) pencil icon.

Click **Submit**.The request is submitted and assigned to you. From here, use the buttons at the bottom of the page to either go back to your worklist or view the request.

Click **View Request** to view the request in the CPV.

Click the ![](<../../../../.gitbook/assets/image (498).png>) folder icon on the Notes and Attachments side bar to view the attachment.On the Attachments tab, the fax is attached.

Click the ![](<../../../../.gitbook/assets/image (501).png>) Home icon to return to your worklist.

Since the request was assigned to you, it displays in your worklist.

Click the **Faxes** tab, and then click the **Processed** tab.Note that the fax used on the new request has been moved to Processed.
