The Care Action Plan Report summarizes a person’s programs and any plans of care built within the programs. This user guide explains how to generate a Care Action Plan Report.

When you need a hard-copy summary of a member’s care plan history.

The Care Action Plan Report contains PHI. Follow your organization’s practices and policies for handling sensitive data responsibly.

From the CPV, click **Print**, and then click **Print** **Care Action Plan Report**.

Select the programs you want to include in the report from the **Programs - Plan of Care** menu.

You can select one or multiple programs.To include all programs in the report, click **Select All**. Click it again to deselect all programs in the list.

To include person alerts in the report, select **Include Person Alerts into the report**. Otherwise, deselect it.

To include notes in the report, select **Include Notes into the report**. Otherwise, deselect it.

Click **Print**.\

Follow your standard procedures for printing to PDF or to a physical printer.
