Manual Medication Entry

Overview

Manually entering medications help maintain an accurate, up-to-date list of prescription and non-prescription medications on the person’s record.

* Access to Care Management or Utilization Management * Permission to view Health Summary * Permission to manual medication function

When the medications the person has indicated they are taking are not yet included in claims data or when there is a discrepancy.

* There is no systematic medication reconciliation process for medications that have been manually entered. Claims data received at a future point may contain a medication that was added manually. * If a manually entered medication is a duplicate of one in the Claims section, delete it to avoid duplicating data in the person’s record. * If a medication is added to the list in error, you can delete it from the list. * Use custom medication names to capture Route information. * Status should be discontinued if the to date is in the past. * There is no audit history on medication additions and edits.

Accessing Medication Records

Starting point is any page where the Health Summary panel is available.

Open the **Health Summary** panel located on the far right.

(_Optional)_ Click ![](<../../.gitbook/assets/2 (2).png>) **Enter Full Screen** to open the Health Summary in full screen view._Note:_ The same medication functionality is available regardless of which view you select.

Click **Medications** beneath the Recent Activity panel.

The Medications List contains two sections:* **MANUAL:** Medications added to the list manually. * **CLAIMS:** Medications received via Claims ingestion.

Adding a Medication

Before you add a new medication, first verify that the medication is not in the list of Claims-based medications or manually entered medications.

Click **+** under the Search entry field.

Select **Medication Status.*** **Active** indicates the person is currently taking the medication. * **Discontinued** indicates the member is no longer taking the medication.

Enter one to several keywords in the **Medication** field.

_(Alternate)_ Enter a custom medication name by typing it into the **Medication** field.

(_Optional)_ Change the **From Date** by typing in a new date or selecting from the calendar picker. This cannot be in the future.

(_Optional)_ Enter the **To Date** to capture the date the member stopped taking the medication.

Select a **Frequency.**

Click **Add Medication.**

_(Alternate)_ Click **Cancel** to return to Health Summary and discard the medication.

Editing a Medication

Edits can be made to manual medications. Consider the status when added a To Date.

Click the pencil icon next to the medication needing changes.

Make the necessary changes.

Click **Save** to complete the changes and return to the Health Summary.

(_Alternate)_ Click **Cancel** to discard the changes and return to the Health Summary.

Deleting a Medication

Manually entered medications can be deleted. There is no audit history captured for this action, so it is deleted and not able to be restored.

Click the ![](<../../.gitbook/assets/8 (2).png>) **Delete** icon next to the medication to be removed.

Click **Delete** to permanently remove the medication.

(_Alternate)_ Click **Cancel** to return to the Health Summary without removing the medication.
